Patient From Hell Explodes at Emergency Room… It Backfires
HOSPITAL ARREST TURNS INTO A DEADLY GUNFIGHT — BODYCAM CAPTURES THE ENTIRE ESCALATION
On April 12, 2021, a medical emergency in Westerville, Ohio, developed into a fatal armed confrontation inside the emergency department at Mount Carmel St. Ann’s Hospital.
The man at the center of the incident was 27-year-old Miles Jackson. According to the Ohio Bureau of Criminal Investigation (BCI), Jackson was initially found unresponsive and was transported to the hospital after EMS treated him for a possible overdose. Officers later learned that Columbus police had felony warrants for his arrest. When officers attempted to take him into custody, a struggle developed inside an emergency-department room.
WHAT BEGAN AS A MEDICAL RESPONSE WOULD END WITH GUNFIRE INSIDE A WORKING HOSPITAL.
1. A MEDICAL EMERGENCY BRINGS POLICE AND EMS TO MILES JACKSON
The chain of events began before Jackson ever entered the hospital.
BCI's investigation states that EMS personnel responded after Jackson was found unresponsive in a parking lot during a suspected overdose. Narcan was administered, and Jackson regained consciousness while being transported to St. Ann's.
At this stage, the priority was medical care.
There was no hospital gunfight.
There was no armed standoff.
Jackson was a patient being transported for evaluation.
THE EVENTS THAT WOULD EVENTUALLY TURN THE EMERGENCY DEPARTMENT INTO A CRIME SCENE HAD NOT YET BEGUN.
2. JACKSON ARRIVES AT MOUNT CARMEL ST. ANN'S
Jackson was taken into the emergency department at St. Ann's.
A nurse interviewed during the investigation said Jackson was alert and cooperative when she assessed him. She also recalled that he had been at the hospital earlier that day.
Police subsequently determined that Jackson had outstanding felony warrants through the Columbus Division of Police.
Columbus officers therefore came to the hospital to take him into custody.
That changed the purpose of the police presence.
THE ENCOUNTER HAD STARTED WITH MEDICAL CARE. IT WAS NOW BECOMING AN ARREST.
3. OFFICERS GATHER INSIDE THE EMERGENCY DEPARTMENT
The bodycam footage provides an unusually detailed view of the hospital environment.
Officers can be seen around the emergency department, including near the nurses' station and treatment rooms.
The frame you supplied captures exactly this setting: uniformed officers inside the hospital while medical operations continue around them.
Nothing about the environment resembles the location of a conventional police confrontation.
There are hospital doors, nurses' stations, treatment equipment and patients nearby.
BCI later reviewed both body-worn camera recordings and hospital surveillance video while reconstructing the incident.
THE BODYCAM WOULD SOON RECORD A DEADLY CONFRONTATION FROM ONLY FEET AWAY.
4. OFFICERS PREPARE TO TAKE JACKSON INTO CUSTODY
Inside the treatment room, officers began preparing Jackson for transport.
Published accounts of the released bodycam footage show an officer explaining that Jackson's belongings needed to be collected. Jackson was then asked to stand so officers could search him.
The interaction initially appears manageable.
But the search changed everything.
Officers discovered ammunition.
They then detected a firearm concealed on Jackson.
Police1's review of the bodycam reports that the weapon was concealed in his groin area.
A ROUTINE SEARCH BEFORE TRANSPORT HAD JUST REVEALED A CONCEALED FIREARM.
5. THE ARREST SUDDENLY BECOMES A PHYSICAL STRUGGLE
Officers attempted to gain control of Jackson.
Bodycam captured commands for him to give officers his arm and release his grip.
Jackson can be heard saying that he intended to comply, but the struggle continued.
An officer deployed a Taser.
Jackson and an officer went toward the floor.
The hospital bed and surrounding medical equipment limited everyone's ability to move freely.
Then came the sound that transformed the confrontation.
A GUNSHOT ERUPTED INSIDE THE TREATMENT ROOM.
According to reporting based on the footage, Jackson discharged a round through his clothing during the struggle.
6. OFFICERS REALIZE THEY ARE NOW FACING AN ARMED THREAT
The first gunshot immediately changed the officers' tactics.
One officer moved away from the immediate struggle while another drew his firearm.
BCI's investigation later documented a non-law-enforcement firearm at the scene and examined firearms, casings and projectiles as part of its forensic work.
Police1 reported that an officer attempted to fire during the initial struggle but experienced a handgun malfunction.
This was no longer simply an arrest involving physical resistance.
There was now a firearm in play inside an emergency department.
OFFICERS, SECURITY PERSONNEL, MEDICAL STAFF AND OTHER PEOPLE INSIDE THE HOSPITAL WERE POTENTIALLY IN DANGER.
7. THE TREATMENT ROOM BECOMES THE CENTER OF A STANDOFF
Officers repositioned while Jackson remained inside the treatment area.
What followed was a tense period of repeated commands.
Police1's account of the recording reports that officers gave Jackson more than 85 commands over roughly three minutes, repeatedly telling him to release the firearm and show his right hand.
The physical environment made the situation particularly difficult.
Officers could not simply rush through the doorway.
They knew a gun had already been discharged.
They also had to consider everyone else in the hospital.
FOR SEVERAL MINUTES, A SMALL EMERGENCY-DEPARTMENT ROOM FUNCTIONED LIKE THE CENTER OF AN ARMED STANDOFF.
8. ANOTHER SHOT IS FOLLOWED BY A VOLLEY OF POLICE GUNFIRE
Officers eventually attempted another Taser deployment.
Then another gunshot was heard.
The response was immediate.
The bodycam recorded a burst of gunfire as officers and armed hospital security personnel engaged Jackson. Police1 reported that one officer said he had been hit; his ballistic vest had been grazed.
BCI's official records confirm that Columbus police officers and armed St. Ann's security officers discharged their duty weapons during the incident.
The contrast is striking.
Minutes were spent issuing commands and trying to resolve the confrontation.
The final exchange happened in seconds.
THE STANDOFF HAD BECOME A FULL-SCALE SHOOTING INSIDE THE ER.
9. OFFICERS RUSH BACK IN AND THE HOSPITAL BECOMES A CRIME SCENE
Once the gunfire stopped, officers moved back into the treatment area.
Jackson was secured, placed onto a hospital bed and transferred to a trauma area for treatment.
He died from his gunshot wounds.
No St. Ann's hospital employee, Columbus police officer or Westerville police officer was reported injured.
The scene then had to be preserved for investigators.
Witnesses were separated.
The emergency-department room was secured.
BCI agents took custody of the scene.
A place designed to save lives had suddenly become the location of a major officer-involved-shooting investigation.
THE GUNFIRE HAD ENDED, BUT THE WORK OF DETERMINING EXACTLY WHAT HAPPENED WAS JUST BEGINNING.
10. BODYCAM AND FORENSIC EVIDENCE RECONSTRUCT THE FINAL MINUTES
BCI conducted an extensive independent investigation.
The agency says investigators processed the scene, interviewed 28 law-enforcement officers and 17 civilian witnesses, analyzed firearms, cartridge cases and projectiles, reviewed bodycam and surveillance recordings, and examined relevant personnel and training records.
Together, that evidence established the broad sequence:
JACKSON WAS FOUND DURING A MEDICAL EMERGENCY.
EMS TRANSPORTED HIM TO ST. ANN'S.
POLICE LEARNED THAT HE HAD OUTSTANDING FELONY WARRANTS.
COLUMBUS OFFICERS ARRIVED TO TAKE HIM INTO CUSTODY.
OFFICERS BEGAN SEARCHING HIM.
AMMUNITION AND A CONCEALED FIREARM WERE DISCOVERED.
A PHYSICAL STRUGGLE FOLLOWED.
A TASER WAS DEPLOYED.
GUNFIRE ERUPTED INSIDE THE TREATMENT ROOM.
OFFICERS REPOSITIONED AND ISSUED REPEATED COMMANDS.
THE FINAL CONFRONTATION ENDED IN AN EXCHANGE OF GUNFIRE.
JACKSON WAS FATALLY WOUNDED.
The significance of the bodycam is that it captures far more than the final shots.
It records the transition from patient care to custody, from custody to a search, from the search to a struggle and ultimately from that struggle to an armed confrontation.
That context matters because a still image of officers standing inside a hospital tells almost nothing about how the encounter reached its fatal conclusion.
THE MOST STRIKING PART OF THE FOOTAGE IS HOW QUICKLY AN APPARENTLY CONTROLLED HOSPITAL ENCOUNTER CHANGES ONCE THE CONCEALED FIREARM IS DISCOVERED.
CONTINUE WATCHING
It begins as a seemingly controlled encounter inside an emergency room—but the moment officers start searching Miles Jackson, everything changes. Follow the full footage to see how the arrest attempt escalated into a deadly armed confrontation inside St. Ann's Hospital.
